Working hours aside, these are good rules. I would potentially add one more, well, extend the 2 dev rule to include someone from another team as well. This shares knowledge between teams, and gets someone with a completely objective set of eyes to have a look at the code. I also agree that managers have no business approving code. Either they're on the IC track or not.
